The Community Health Improvement Plan, or CHIP, is an action-oriented strategic plan that outlines the priority health issues, how these issues will be addressed, including strategies and measures, to ultimately improve the health of the community. The long term goals identified in the Community Health Assessment (CHA) are to make measurable progress towards, reducing the burden of chronic disease caused by obesity, improving access to affordable quality nutritious food, increasing accessibility to community resources via safe, active transportation, and expanding access to patient-centered medical homes for all persons.
